Empowering Teamwork at BP PMF

On August 12, a Cargo Carrying Unit (CCU) transporting a radiator was sent from an offshore asset to Danos' team at BP's PMF to be repackaged before sending it to the vendor for repair. After offloading and landing the CCU container holding the radiator, the forklift operator communicated to his fellow co-workers to be careful when opening the CCU.

Upon opening the CCU, the crew observed the radiator resting diagonally inside the box, which could create problems from load-shifting and the team's ability to handle the cargo. The crew stopped the job and left the box closed and secured while they consulted Danos HSE and leadership.

The following day the team regrouped with HSE and Danos SLC to discuss potential options for safely offloading the radiator. A Danos employee asked if the vendor had the equipment and resources to safely offload the radiator. Since the vendor is the subject matter expert on the radiator and its handling, the group collectively agreed and called the them.

After exchanging conversations and pictures, the vendor agreed they could safely offload the radiator with the equipment and resources at their location. The operations crew was able to eliminate the need for PMF personnel to remove the radiator. The crew used a hierarchy of controls, communication skills and a “thinking outside the box” mentality to come up with a viable solution.

The crew was recognized by site leadership, and this observation was shared with management at Danos and BP.
