Stop Work Authority Challenge and Debate

Pat Gipson, FPSO Turritella mechanic technician, was working with a crew to change out a hydraulic accumulator on an operating pressure system. Once the accumulator was installed and the crew filled it with nitrogen to the correct pounds per square inch (approx. 3000 PSI), the crew noticed a leak on one of the oil-side fittings of the accumulator.

Pat used stop work authority and explained that in order to proceed, they needed to depressurize the accumulator bottle and redo the oil fittings with the correct lubricant. The crew was reluctant at first, but all agreed that this was the best course of action after Pat explained the consequences of either integrity failure, blow out or line of fire.

Pat's vigilance and foresight, his stop work authority, challenge and debate and plan of action prevented integrity issues for our customer, and most importantly, protected all team members involved.
